Guidance Notes and Preparation Forms

Quest and Quest Plus Assessment

Quest and Quest Plus will be running until 31st December 2025, module guidance for our new Quest Assessment will be available in due course.

Facilities can choose to go through Quest, which is a Mystery Visit and 1-Day Assessment or Quest Plus which is a Mystery Visit and 2-Day Assessment.

Below is a table of our Quest Core Modules.

To access the Guidance Notes and Assessment Preparation Forms simply click on 'Download' for the corresponding module.

For all Quest Assessments, it would be beneficial to complete the preparation forms in advance - in particular the Compliance Declaration. 

Quest for Facilities (Plus Modules) - Available until 31st December 2025

Please note:

• "Planning to Improve" is already selected for anyone completing Quest Plus, and you are able to select 4 additional modules.
• SPLUS28 Swim England Learn to Swim and GPLUS38 Exercise Referral Standard are larger modules and each will take up 2 module choices, if selected.
• If you wish to select 'SPLUS28 Swim England Learn to Swim' we need to know within 7 days of receiving your purchase order number, as this module requires an assessor with specialist training
• SPLUS28 Swim England Learn to Swim cannot be selected with SPLUS20 Swimming Lessons.
• If we do not receive your modules choices for Quest Plus two weeks before your assessment date, they will be chosen for you using our generic lists.
